To expedite the registration process, please access the Forms link to download our forms. You may fax the completed forms to (915) 838-7936 with a copy of your insurance information. You may also bring these forms with you on your appointment.

To help keep our billing costs and our fees as low as possible, we ask that you pay for office services and/or co-payments required by your insurance plan at the time of service.

We gladly accept MasterCard and Visa. We will file all your insurance claims for reimbursement.
